This is a list of the medicals required to fight Pro MMA at Mohegan Sun (CT). The list contains notes to help make sure you get the proper tests and complete the forms accurately. The list of the medicals along with notes about each medical can be downloaded below in PDF format. The forms for each medical can be downloaded in the next section below. List of required medicals:
- – Negative HEP-B Sag, HEP-C AB, HIV tests dated within 6 months of fight night
- – Physical dated within one year of fight night
- – EKG and interpretation form (no time requirement)
- – Eye exam done by an ophthalmologist, dated within one year of fight night
- – CT Scan or MRI dated within one year of fight night
- If CT Scan or MRI is older than a year, a neuro exam, done within one year of fight night, can be submitted WITH the old CT Scan or MRI results.
